Brooke Nash MPH, CCHW

Our Patient Educator and Certified Community Health Worker (CCHW) empowers families by providing education, personalized support, and connections to valuable community resources. Brooke works closely with patients, families, and our healthcare team to promote healthy development, answer questions, coordinate care, and provide guidance on topics ranging from newborn care and childhood wellness to nutrition, safety, and behavioral health. Through educational programs and community outreach, she helps families feel informed, supported, and confident in caring for their children.
